Cone Health & Proficient Health Collaborate to Bring Best in Network Fidelity and Care Coordination to Market.
Cone Health Ventures is excited to announce the formation of a strategic initiative with Proficient Health. This initiative will focus on developing and delivering a best-in-class suite of tools that will enhance outcomes and margin for value-based organizations and physician networks.
Triad Health Network (THN), Cone Health’s ACO and one of the largest, most successful in the country, is currently using PH Exchange, a Proficient Health referral management solution, to seamlessly connect providers using disparate systems across nearly 200 locations. PH Exchange allows THN practices to create, track and manage referrals in a centralized system that can integrate directly with each EHR instance. Practices and providers have full visibility into referral statuses and can easily receive follow-up notes or other clinical information.
Proficient has also integrated another product, PH Capture, into PH Exchange to allow THN practices to assign all incoming faxes quickly and easily into the correct workflow. The fully integrated solution streamlines information exchange from provider to provider and eliminates the need for paper faxing, printing, and scanning.
Cone Health Ventures identifies entrepreneurial companies with solutions that address the greatest needs and top strategic aims of Cone Health. By negotiating win-win agreements to align the interests of all parties, Cone Health Ventures accelerates commercialization into the broader healthcare market to create value for the companies, Cone Health, and our patients.
